Before requesting a report, please make sure you are familiar with our reporting process:

• To protect client’s private data and to comply with HIPAA laws, only confidential user data will be provided.  This means only an anonymous ID # will be provided and names and other common identifiers will be excluded from the reports
• We provide data on each user within your organization.  The current data we do report on is:
• Confidential ID #
• Initial Weight
• Current Weight
• Minutes of Activity
• Participation
• Reports are generally ran and sent back to your organization within 3-5 business days.  Occasionally we may have it sooner or a day or two later, depending on our current workload and the number of requests
• Generally reports are provided on a quarterly basis
• If your organization requires more frequent reports or further info, other than the above fields we currently report on, you may let us know below.  The above data and quarterly reports, are provided at no charge.  In some cases, especially if a new or custom reporting process is to be programmed by our development team, there may be extra costs.  We will notify you of these costs for approval prior to beginning any custom requirements.
